New Patient Registration
If you wish to register with Trescobeas Surgery, please first confirm that you are living within the practice area. This area is detailed on the map on the above tab by a red dashed line.
Please then complete the NHS GMS1 form and our New Patient Questionnaire and send or hand both in to the surgery. These forms can be downloaded from the above "Registering Online" tab.
If you would like to access online services please come into the surgery with a current email address and a form of ID, such as a birth certificate, driving licence or passport, and we will be able to set this up. Please note ID is mandatory before we can allow access.
If you are taking any medications it would be helpful to bring in a copy of your current prescription counterfoil.
Your registration will be uploaded to the Local Health Authority. Your notes will then be called form your old surgery and sent to us. Please note this can take some time.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
