Poster of their service

Discharge Liaison Nurse (DLN)
This service is designed to look after patients discharged from hospital. Many come out confused about new medicines, unsure about their normal medication, unsure about any follow up requirements or unable to cope alone at home. The DLN is tasked to contact the patient and ensure that all these issues have been addressed. She works closely with the District Nurses, Social Services and Community Matron all of whom may have some input into the patient’s care.

They are based are Falmouth Hospital.
