The nurses are responsible for the care of those patients who are unable to access the surgery due to their medical condition – the housebound and those recently discharged from hospital that need further care at home. They are located in Falmouth Hospital and can be contacted directly on 01326 430075.
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
They hold Child Health Surveillance clinics on Wednesday afternoon most weeks. They can be contacted direct on 01326 430050.
Andrew Taylor and Nicola Child are our new Musculoskeletal (MSK) Practitioners. Both Andrew and Nicola have extensive expertise in the clinical assessment, diagnosis and management of MSK (muscle or joint) conditions. They see patients, aged 16 and over, with (suspected or diagnosed) MSK conditions, as the first point of contact, without need for a GP referral. Appointments can be booked directly by contacting the practice reception team.
Discharge Liaison Nurse (DLN)
This service is designed to look after patients discharged from hospital. Many come out confused about new medicines, unsure about their normal medication, unsure about any follow up requirements or unable to cope alone at home. The DLN is tasked to contact the patient and ensure that all these issues have been addressed. She works closely with the District Nurses, Social Services and Community Matron all of whom may have some input into the patient’s care.
They are based are Falmouth Hospital.
